Who fills it in
The food business operator, a person or a company, before the premises open, and again when the business or the operator changes. The Deputy Ministry of Tourism and the municipal premises licence both ask for this certificate, so it usually comes first.

Common mistakes
- Bringing an old water test: the analysis must be from the last month and from the kitchen's cold-water tap.
- Applying after opening: the certificate comes before the premises serve food, and other licences ask for it.
- Leaving out the person in charge when someone other than the applicant runs the kitchen.
- Using another municipality's form or channel: apply to the Health Service of the municipality where the premises are.
