Who fills it in
EU citizens who stay in Cyprus longer than three months, within four months of arrival. Each family member who is also an EU citizen files their own application. Non-EU family members use a different form (MEU2).
What goes with it
- Copy of a valid passport or ID card, with the original shown
- Part III of the form signed and stamped by your employer, and a salary certificate where applicable
- Social Insurance registration certificate and detailed statement of contributions (self-employed: last four months)
- Form A1 and Plan A health insurance if you pay social insurance in another EU country
- Title deed, or rental contract stamped and with certified signatures, plus rent receipts
- Original utility bill (EAC or water) in your name
- Marriage, divorce and children's birth certificates where relevant, apostilled and translated
- Students, pensioners and dependants: proof of income or study and Plan A or GESY cover, as the supporting documents list sets out
- Declaration of Mailing Address

Common mistakes
- Leaving Part III unsigned or without the company stamp. The supporting documents list asks for it signed and stamped.
- Waiting too long. The form says you apply within four months of arriving in Cyprus.
- A rental contract without the stamp duty mark or certified signatures, or no utility bill in your name.
- Using a different passport or ID number from the document you bring to the appointment.
- Forgetting the Declaration of Mailing Address, which the form asks you to attach.
- Foreign marriage or birth certificates without an apostille or a Greek or English translation.
Questions
Where can I download the MEU1 form?
From the Migration Department's MEU1 forms page on gov.cy. The current file is a Word document called MEU1B, dated 9 March 2026. The same page has the supporting documents list, the Declaration of Mailing Address and the Authorisation of Representative.
Is it MEU1 or MEU1B?
The service is still called MEU1, but the form itself is printed with the code MEU1B. It is the same Yellow Slip application.
Can I fill in MEU1 in English?
Yes, the labels are in Greek and English and the form does not ask for Greek. Supporting documents must be in Greek or English, or come with an official translation.
Is the form free?
Downloading the form is free. When you submit it you pay €20 for the EU citizen and €20 for each family member's application, as printed on the form.
Can someone submit MEU1 for me?
A representative with a signed Authorisation of Representative can handle the file and collect the certificate. You still need to attend in person so that your photo can be taken.
