Who fills it in
EU, EEA and Swiss citizens, and their family members who are also EU citizens, who have lived in Cyprus legally for five continuous years, usually with an MEU1 registration certificate. Non-EU family members use the MEU3 form for non-EU family (MEU3B-1).
What goes with it
- Copy of a valid passport or ID card, with the original shown
- Copy of your registration certificate (MEU1, the Yellow Slip)
- Marriage or civil partnership certificate, apostilled and translated if issued abroad (original shown)
- Divorce, dissolution or death certificate where relevant (original shown)
- Children's birth certificates, certified and translated, where relevant
- Monthly Social Insurance contribution statement and tax returns for the last five years (pensioners too)
- Proof of stable income for the last five years (bank activity of the last quarter, remittances, pension, rents, dividends, interest)
- Electricity and water bills in your name for the last five years
- Title deed, or rental contract stamped and with certified signatures, plus original rent receipts and a utility bill in the tenant's name
- Minors: the separate list for children (parents' permanent residence documents, parental consent or court decision, school attendance for five years)
- Declaration of Mailing Address

Common mistakes
- Applying before five continuous years of legal residence in Cyprus, the condition stated on the form.
- Leaving Part III unsigned or without the employer's stamp.
- Bringing a contribution statement for less than five years. The supporting documents list asks for five years, month by month, and five years of tax returns.
- Missing utility bills. The list asks for electricity and water bills in your name for the last five years.
- Filing this form for a non-EU spouse or child. They use the MEU3 form for non-EU family (MEU3B-1).
- Forgetting the Declaration of Mailing Address, which the form asks you to attach.

Where can I download the MEU3 form for EU citizens?
From the Migration Department's MEU3 forms page for EU citizens on gov.cy. The file is a Word document coded MEU3B, dated 9 March 2026, with the supporting documents list and the Declaration of Mailing Address.
Which MEU3 form do I need?
This one if you are an EU, EEA or Swiss citizen or an EU family member of one. A non-EU spouse, child or parent uses MEU3B-1, the MEU3 form for non-EU family, which leads to a Permanent Residence Card.
How much does it cost?
The form says you pay €20 for the EU citizen and €20 for each family member's application when you submit it.
Can someone submit it for me?
You need to attend in person so that your photo can be taken. The form says the certificate is handed only to you or a duly authorised representative.
